2026 Drivers Training Re-Entry Application

Welcome—we’re glad to hear you’re interested in continuing your driver training as you work toward earning your driver’s license.

We are pleased to have the opportunity to work with you. Please understand that our instructional hours are limited, and participation in this program requires commitment and responsibility. If your goal is to obtain your driver’s license, it is essential that scheduling and attendance are taken seriously.

By submitting the form, you are confirming that you will arrive to lessons prepared, on time, and ready to drive, and that you will minimize cancellations. As a small nonprofit organization, last-minute cancellations place a strain on our limited resources. Missed or canceled lessons still count toward your allotted training hours, and many students have lost valuable driving time and were unable to test due to cancellations.

The form below must be completed and a photo of your active permit must be submitted before we can request driving hours on your behalf. Please email a picture of your permit to: eric.changedrivers@gmail.com or text it to 651-357-7791.

If your permit has expired or you have never received one, please contact me directly—I can help guide you through the steps to obtain or renew it.

Once your form and permit photo are received, I will contact the county to request hours. After approval, you will be placed in line for the next available instructor. Please note that this process may take some time depending on approvals and instructor availability.

We have a strict 24-hour cancellation policy. Failure to provide adequate notice will result in the loss of lesson hours and may lead to termination from the program if it occurs repeatedly. Please respect our instructors’ time.

For vehicle availability and coordination, it is sometimes easier for us to schedule your driving test. Would you like us to assist with scheduling your driving test?
